Personal Survey

There are six behavioral areas to survey personal strengths and weaknesses to improve.

Measurable objectives are included to provide valid reasons for taking the personal survey.

Measurable Objectives

• raise your level of awareness.
• move from unconscious to conscious awareness.
• record general and specific responses.
• assign a Likert Scale rating of 1 low to 5 high for your personal selections.
• make an honest appraisal of your talents, abilities, and skills.
• learn your awareness level is associated with your personality.

Open the PDF and select PRINT to a letter sized paper with room to place your answers.

Print out two blank copies – one for yourself. Then after you complete your survey, use the other blank copy to give to someone who knows you well enough for several years and has the time to complete the survey.
